Please have your family doctor complete this Referral Form. Once we receive the referral, we will send you an intake form and booking link.

For a Pap test, please click here. Please note: Anyone with a cervix, including women and TTGD (Two-Spirit, transgender and gender diverse) people, between the ages of 25 and 69, should screen for cervical cancer every three or five years (unless recommended by your doctor).
For all other episodic health concerns, please fill out our New Patient Intake Form and we will contact you within one week to schedule an appointment.
For Trans Care Surgical and Hormone Readiness Assessments, we currently have a waitlist. Please complete the New Patient Intake Form and we will contact you to add you to the waitlist.
If you are a current family practice patient, you are welcome to book with our nurse regarding any health concern you may have. Our nurse typically has earlier availability than our doctors. Please use the link here.
